Workers’ Compensation Insurance
All on the job injuries must be reported immediately, or within 24 hours, to your immediate supervisor or a supervisor at the time of injury. If you are a campus employee, please also be sure to report your injury to the campus nurse so the extent of your injury can be assessed. For a medical emergency please call 911 and seek treatment.
1. Complete Online First Report of Injury - for use by LWISD employees. If online reporting is not available, please contact Payroll/Benefits at ldeleon@lwisd.org or call extension 1049.
2. A Preauthorization approval form is required to be sent prior to being seen by an Approved Provider please call Renee DeLeon at extension 1049 or 1000 for the main desk. If authorization is needed after hours, please email Shelley Miller shmiller@lwisd.org.
3. Look up current Worker's Compensation Approved Providers
4. An LWISD employee who receives treatment under Workers' Compensation benefits must complete Acknowledgment of Alliance Form - for use by LWISD employees.
Lost Time:
If an individual loses time beyond the day of injury, please complete the Worker's Compensation Election Form and contact Renee DeLeon ldeleon@lwisd.org, Ext. 1047 in the HR/Benefits Department in order to complete the necessary paperwork pertaining to time away from work. This ensures that an individual is compensated accordingly.
Leave taken due to a work-related injury runs concurrently and could be designated as FMLA leave, temporary disability leave, and/or assault leave, as applicable. Please Renee DeLeon ldeleon@lwisd.org, Ext. 1047 to discuss leave of absence eligibility and options.
Worker’s compensation is not a form of leave. The Workers Compensation law does not require the continuation of the district’s contribution towards health insurance. Should you have any inquiries regarding benefits,
