  • Proof of Residency
    Acceptable documents to establish proof of residency include:

    • current electric, gas, or water bill (Note:cell phone bills are NOT acceptable)
    • current property tax statement
    • current lease or rental agreement
    • current deed of sale for purchase of a home

    Special Circumstances

    The following special circumstances require additional documentation. If you have questions about which form to use, please contact your student's campus. Contact information is available on the Campuses General Information page.

    • This form must be completed for a student who lives within the boundaries of Lake Worth ISD, but desires to attend an a campus outside of the student's assigned attendance zone. The form must be completed in it's entirety before it will be approved.
       
    • This form must be completed for a student who lives outside the district, but requires before and/or after school care from a grandparent who lives within the Lake Worth ISD boundaries. The form must be completed in it's entirety before it will be approved.
       
    • Family in Residency Request -  in English    en Español
      This form must be completed for a student whose family is temporarily living with a Lake Worth ISD resident. The form must be completed in it's entirety before it will be approved.
       
    • This form, to be completed for a student living with a non-parent custodian, provides the custodian rights to make educational decisions for the student and access to the student's school records.
       
    • Unhoused or Homeless Students

    To be considered current, documents must be dated within the last 30 days, with the exception of tax statements which must be from the most recent year.
