Lake Worth ISD offers a variety of insurance policies to our employees.
Policies are independent of one another, meaning that an employee does not have to enroll in one policy in order to enroll in any other policy.  Detailed information can be found by reading the information at the links below which are designated by the Ø symbol.                                                      
Note: If the terms of this benefits summary differ from your policy, the policy will govern.

Ø2012-2013 Lake Worth ISD Benefits Guide: Contains information about the online enrollment process as well as summaries of policies offered for the 2012-2013 plan year.
 
Ø TRS-ActiveCare Enrollment Guide 2012-2013 Health Plans: Contains information about the TRS-ActiveCare health insurance plans available to all employees of Lake Worth ISD who are scheduled to regularly work at least 15 hours per week. *The rates contained in this document are the monthly premiums charged by TRS-ActiveCare.  Lake Worth ISD pays $225.00 per month towards the monthly premiums of those enrolled in the TRS-ActiveCare plans.  For the Employee Paid portion of the premiums, please see page 13 of the 2012-2013 Lake Worth ISD Benefits Guide (above).   TRS-ActiveCare Application Form

ØApproved 403(b) Annuity Vendors: Use this link to find a list of approved vendors in the Lake Worth ISD 403(b) Plan Document and forms for employees who would like to start/change/terminate a 403(b) annuity.  For more information about annuities, please contact one of the approved vendors on the list. 
*Note: 403(b) Annuities are not the same as your Teacher Retirement System of Texas retirement contributions.  403(b) annuities are additional monies taken out on a pre-tax basis from the employee's payroll check.  Please make sure to thoroughly review the contracts when signing an annuity as they are contracts between the employee and their annuity vendor.  Lake Worth ISD is not responsible for any fees and/or penalties which might be imposed under your annuity contract.
