Miller Elementary Registration

RETURNING STUDENTS in K - 4th grade: Must provide documentation by September 1st in order to remain enrolled in the district.

Returning students need to provide a proof of residency: Utility bill with current address. Cell phone bills are not accepted.

NEW STUDENTS in K - 4th grade - For new students, the following documents are required:

Birth Certificate
Social Security Card
Immunization Records
Proof of residency: may be a telephone bill (not cell), gas bill, electricity bill, or water bill, lease agreements, rent receipts, or proof of mortgage payments
Records from previous school

PREKINDERGARTEN 3 YR AND 4 YR STUDENTS -All new and returning PK students need to provide the following:

Birth Certificate
Social Security Card
Immunization Records
Proof of residency: may be a telephone bill (not cell), gas bill, electricity bill, or water bill, lease agreements, rent receipts, or proof of mortgage payments
Copy of 2005 income tax return
Copies of last two paycheck stubs for all working members of the household
